Acomplia / Rimonabant - Technical Information
Acomplia was recently approved in the UK as a controlled medication
and is under application in the US. The active ingredient in Acomplia is
rimonabant, which is a CB1 endocannabinoid receptor antagonist.
(The endocannabinoid system is in the part of the brain that regulates
metabolism, energy levels and appetite).
Rimonabant prevents the endogenous cannabinoid binding to CB1 receptors
in nerve cells. These CB1 receptors, when activated, stimulate appetite.
Hence this drug blocks the activation of these CB1 receptors and thus
reduces appetite.
The CB1 endocannabinoid receptors are also involved in the mechanism of
other addictions such as smoking, and it may be in time that this drug
is approved as a treatment for addictions. Certainly, the effect on the
endocannabinoid receptors has a benefical effect on blood glucose and
lipid levels - a more beneficial effect than would be expected from
weight loss alone.
Side Effects
All drugs have side effects, and Acomplia is no exception. Some of
the side effects are beneficial - such as the altered blood lipid
profile. However, there are other known side effects that may influence
whether or not you continue to take the drug.
Nausea and vomitting
Nausea, vomitting and other gastrointestinal disorders were relatively
common side effects of taking Acomplia.Disorders of the nervous system
This category includes headaches, faintness and feeling
dizzy.Psychiatric disorders
Including mood swings, anxiety, depression, irritability, sleep
disorders and other similar disorders. If you already have a history of
mental health disorders, it would be highly adviseable to mention this
to your doctor before obtaining this drug. The table attached is an
indication of the relevant frequency of side effects when taking 20mg
Acomplia daily, compared to the placebo.

Contraindications
Acomplia should not be taken, or should be taken only under
exceptional circumstances under your doctor's direct guidance, in the
following circumstances:
1. Anyone who exhibits hypersensitivity or an allergic reaction to
the drug (it should never be used in these circumstances).
2. Anyone with an impaired liver function. The effects of Acomplia
have not been studied in patients with liver damage.
3. Anyone with impaired kidney function. It should never be used in
patients with a severly limited kidney function.
4. Anyone over the age of 75. There is no data currently available to
establish safety in this age group.
5. Any children under the age of 18 - there is currently a lack of
data on efficacy and safety for this age group.
6. Pregnant or lactating women - safety in these groups has not been
established. Indeed, current data in rats shows possible damage to the
developing foetus and an inhibited sucking reflex in young rats.
7. There is little data on the safety of Acomplia in patients
suffering from epilepsy and it should be prescribed with caution in
these instances.
8. The Acomplia tablet contains lactose and should therefore not be
prescribed to patients with a lactose intolerance.
9. It should only be used with caution in combination with CYP3A4
inhibitors, such as ketocinazole, ritonavir, clarthromycin and related
drugs.
10. It should not be prescribed to patients with an uncontrolled
psychiatric condition, and is not recommended for use with
antidepressants as there is a lack of data on the safety in these
circumstances.
